How the Post‑19 Transition Process Works

Planning for Post‑19 education is a gradual process that takes place during your child’s final years at school. The aim is to make sure the next placement is right for your child’s needs, abilities, and future goals.

1. Annual Review Discussion

Your child’s Annual Review is the main opportunity to talk about Post‑19 plans.

  • During this meeting, school staff will discuss:
    • Your child’s views and wishes
    • Parent/carer preferences
    • The school’s professional recommendations, if needed
Together, you will begin to consider possible Post‑19 options, such as colleges, sixth forms, or specialist provisions.
For some learners a social care setting maybe considered appropriate for these learners the process would be a "cease of plan" this is can be discussed at the annual review. 

2. Applying to Post‑19 Providers

Parents/carers, together with the young person, are responsible for applying directly to colleges being considered. The college must have an application from the child alongside the consultation from the LA. School staff can offer guidance, but the application itself must be completed by the family.

  • Applications should take into account:
    • Your child’s learning ability and support needs
    • Their predicted or realistic outcomes
    • The type of course or provision that would suit them best

3. Consultation by the Local Authority

Once preferences have been shared, the Local Authority SEND team or Key Worker will consult with appropriate Post‑19 providers.

  • Providers are consulted based on:
    • The young person’s needs
    • Family preferences
    • Location and suitability of the provision

4. Placement Decision

  • After consultations have taken place, a Post‑19 placement is agreed.
  • The named placement will be recorded in Section I of the final Education, Health and Care Plan (EHCP).
  • This confirms where your child will continue their education or supported learning.
