Post-16 Transition
Moving on to Post-16 education is an important stage in a young person’s journey. At Queen’s Croft High School, Post-16 transition planning is carefully structured, personalised and supported in partnership with pupils, families and the Local Authority.
This page explains how the Post-16 transition process works, key timescales and how families can explore appropriate next steps.
Planning for Post-16
Post-16 transition planning is embedded within our careers provision and forms part of our wider Preparation for Adulthood approach.
Planning focuses on identifying provision that best meets a young person’s needs, aspirations and stage of development, while ensuring decisions are realistic, appropriate and sustainable.
The Post-16 Transition Process
Post-16 transition planning takes place through the Annual Review process.
For pupils in Year 11 and Year 14, Annual Reviews are held during the autumn term. During these meetings, the following are discussed:

pupil preferences for Post-16 provision

parental preferences
-
on occasion, the school’s professional view where this differs
-
readiness for different pathways
Following the Annual Review:

Parents, carers and young people apply directly to colleges or Sixth Form providers

SEND key workers consult with Post-16 provisions based on preference and location
-
The Local Authority considers consultations and determines the placement
-
Final placements are confirmed and recorded in Section I of the EHCP
Families are usually informed of Post-16 placements by March.
Exploring Post-16 Options
It is important that families are aware of the range of Post-16 options available. We support this through:

open days and provider events

careers and transition events hosted at Queen’s Croft
-
individual guidance and discussions

signposting to local and regional provision
Where families are unable to attend advertised open days, providers may be able to offer informal visits or discussions.
Post-16 Provision at Queen’s Croft
Queen’s Croft High School offers a Sixth Form provision which includes:

entry level qualifications

work experience and employability learning
-
life and living skills (including our on-site flat)

enterprise opportunities
This option is discussed alongside external Post-16 provision to ensure the most appropriate pathway is identified for each young person.
Transport and Local Authority Considerations
Transport arrangements and Local Authority policies may vary depending on location and type of provision.
Where Post-16 provision is outside the home Local Authority, families will be supported to understand transport arrangements and any associated personal or transport budgets as part of the transition planning process.

We recognise that Post-16 transition can feel complex. Families are supported throughout the process through:
