The Outreach Model

Our outreach service supports schools in Lichfield and Burntwood through the SEND & Inclusion Hub and
LMG. Our model is flexible and adapts to changing systems while maintaining high quality consistent
support. Outreach workers will build strong relationships with the child, school and family to understand
the child’s needs and plan effective interventions.

Who Is It For?

Our outreach offer is designed for pupils in mainstream schools who are facing challenges with learning and
behaviour. Schools benefit from our support to strengthen their SEND provision and promote positive
outcomes for their children.

What We Provide

Our outreach service includes visits to the school and working with school professionals to understand the child’s needs. . A personalised plan will be developed with input from professionals to ensure the child gets the correct support.
  • Bespoke delivery options tailored to school and child needs.
  • Direct Outreach across EYFS, Primary, and Secondary.
  • Bespoke support for inclusive/SEND polices & procedures.
  • 1:1 child support: modelling, mentoring, coaching inclusive practices.
  • Whole-school CPD and staff upskilling.
  • Inclusive pedagogy and practical strategies.
  • Parent/carer liaison and workshops.
  • Transition support for moving to next steps.
  • Signposting to additional services.
  • Specialist support for specific SEN (Autism, ADHD, SEMH)

Outreach Assignment

Support Begins

A Referral is submitted by the school through the SEND & Inclusion Hub, ensuring parental consent is secured.

Referrals are reviewed and approved by the Local Management Group before support begins.

An outreach worker is assigned and contacts the school to plan engagement and support for the child.

The assigned outreach worker begins to support the school.
